Your Community Healthcare Center, LLC (YCHC), is

owned and operated by Nurses from the Nation of Islam, along with other health and clinical professionals.

We are here to serve the community with love, respect,

honor, and care.

​

We accept and assist  individuals in our programs who are

seeking case management, education, training/workshops,  housing, medication-assisted treatment, and social assistance for the betterment of their holistic health.

 

We offer professional services and provide contractual services with various profit and non-profit facilities. 

​

In addition, we contract our professional services with home health care,  long-term care facilities, substance abuse, and mental health centers.​
